Ashgrove Medical Group covers two cluster areas, those being Taff Ely & Cynon South clusters.

Taff Ely Cluster

Taf Ely Primary Care Cluster Services
Taff Ely Cluster is one of 8 Primary Care Clusters and is geographically central to Cwm Taf Morgannwg University Health Board. It serves a GP practice population of around 95,000 with 7 GP practices, namely:
- Taff Vale Practice
- Ashgrove Surgery
- Egwlysbach Medical Practice
- Old School Surgery
- Talbot Green Practice
- Parc Canol Practice
- Taffs Well Medical Centre
The cluster area also includes the following to provide access and services for its patient population:
- 21 community pharmacies
- 13 Dentists
- 9 Opticians
- Community Groups and services represented Interlink.
- Local Authority – social care services

The South Cynon Primary Care Cluster serves approximately 30,955 patients.

South Cynon Primary Care Cluster
Our Community - 10% of the population of Wales live within Cwm Taf, the locality is the second smallest and second most densely populated in Wales. The Cynon Valley (CMO for Wales reports, Public Health Wales data) has high rates of: social deprivation, mental health issues, long-term disability/morbidity, poverty/benefits uptake and chronic illness from legacy heavy industry, particularly mining.

What we are working on?
Care Navigation - The cluster has invested in training for their frontline staff to be able to signpost patients to the most appropriate service. Partners include; Citizens Advice Bureau RCT, Common Ailments Scheme, Optometry, Physiotherapy, Single Point of Access & Interlink (Wellbeing Coordinators).
