The WISE Team supports anyone over the age of 18 who has a chronic health condition such as:

• Chronic Disease Self-Management
• EPP Type 2 Diabetes
• Fibromyalgia Self-Management
• Cancer Thriving and Surviving 

You can self-refer or be referred by your Consultant, GP practice, or any other health professional.
