Access NHS Wales Digital Services with In-Person Verification

If you are unable to verify your identity online using NHS Login, you can complete the Welsh Identity Verification Service (WIVS) Application Form at your GP surgery.

This in-practice verification allows you to gain secure access to NHS Wales digital services, including:

  • Booking appointments

  • Ordering repeat prescriptions

  • Viewing parts of your medical record

What You Need to Do

  1. Collect the Form
    You can request the WIVS Identity Verification Application Form at reception or download it here:

  2. Complete Part A
    Fill in your personal details and read the patient agreement carefully before signing.

  3. Provide Proof of Identity
    Bring one of the following to the surgery:

    • Official photo ID (e.g. passport or driving licence)
      OR

    • A combination of documents and information in your health record to confirm your identity

  4. Practice Staff Verification
    A staff member will review your documents and complete Part B of the form to confirm your identity.

  5. Get Access
    Once approved, your access to the NHS Wales App and other digital services will be activated.
